A Cortland man is dead after State Troopers say his car was involved in several crashes before driving into an SUV in Warren Township early Tuesday.

The Ohio State Highway Patrol tells 21 News that 43-year-old Chad Coffee was driving a hatchback that crossed the center line of Mahoning Avenue NW, running into a Chevy Trax just north of North River Road at around 1:30 a.m.

Coffee died in the crash.  The driver of the Trax, 48-year-old Maurice Douglas of Warren, was flown by helicopter to a Youngstown hospital with injuries that Troopers describe as serious, but not life-threatening.

Troopers say they believe speed and alcohol were factors in the crash.

The Patrol says Coffee was involved in two other accidents before he was killed.

Coffee’s Chevy Malibu went out of control on State Route 5 striking a barrier.  When another driver stopped to make sure Coffee was okay, a trooper said he sideswiped the other car and drove away from the crash.

Not long after, the Patrol says Coffee’s car struck a guardrail on the Route 45 ramp from Route 82 eastbound.

Investigators found a trail of debris from the crashes.

The stretch of Mahoning Avenue was closed until 4:30 a.m.

Troopers say neither Douglas nor Coffee was wearing their seat belts at the time of the crash.
